I hereby give HAE International (HAEi) and its employees, agents, affiliates, and business and other partners (collectively, “HAEi”) permission to store, process, use and disclose the name and email provided in this form.

I understand that my personal information - name and email address- may be stored, used, processed, distributed, and otherwise disclosed for HAEi’s general use or for educational purposes.

I am aware that I can withdraw my consent at any time! All I need to do is send a message to (n.tsutsumanova@haei.org) and let the HAEi team know I no longer wish to receive emails regarding HAEi Advocacy Academy and wish to have my information deleted.

I am aware that HAEi reserves the right to edit or reject my written text or any visual content in the case of it being harmful, harassing, blasphemous, defamatory, obscene, invasive of another's privacy, hateful, or racially, ethnically objectionable, religious, disparaging, relating or encouraging unlawful activity in any manner whatever;

I hereby waive the right to any financial or other compensation for the use and disclosure of my testimonials or images. I release HAEi, and those acting under its authority, from any liability associated with the use, processing, distribution, or other disclosure of my personal information (name and email address).
